- Aadhar Card / Current Aadhar Card
- Electricity Bill
- Submit passport size photographs of all family members mentioned in the application. Sickness/Disability Certificate from a Government Institution/Practitioner
- If there is LPG connection in the house, then a copy of LPG/Gas booklet.
- Collective passport size photograph of the family
- Death certificate, if the head of the family is a widow or there is an orphan child below the age of 21 in the family.
- If the family has agricultural land, then ‘Fard’

- ਅਰਜ਼ੀ ਵਿੱਚ ਦਰਜ ਸਾਰੇ ਪਰਿਵਾਰਕ ਮੈਂਬਰਾਂ ਦੀਆਂ ਪਾਸਪੋਰਟ ਸਾਈਜ਼ ਫੋਟੋਆਂ ਜਮ੍ਹਾਂ ਕਰੋ।
- ਸਰਕਾਰੀ ਸੰਸਥਾ/ਪ੍ਰੈਕਟੀਸ਼ਨਰ ਵੱਲੋਂ ਬਿਮਾਰੀ/ਅਪੰਗਤਾ ਦਾ ਸਰਟੀਫਿਕੇਟ
- ਜੇਕਰ ਘਰ ਵਿੱਚ LPG ਕੁਨੈਕਸ਼ਨ ਹੈ, ਤਾਂ LPG/ਗੈਸ ਬੁੱਕਲੈਟ ਦੀ ਕਾਪੀ।
- ਪਰਿਵਾਰ ਦੀ ਸਮੂਹਿਕ ਪਾਸਪੋਰਟ ਸਾਈਜ਼ ਫੋਟੋ
- ਮੌਤ ਦੇ ਸਰਟੀਫਿਕੇਟ, ਜੇਕਰ ਪਰਿਵਾਰ ਦਾ ਮੁਖੀ ਵਿਧਵਾ ਹੈ ਜਾਂ ਪਰਿਵਾਰ ਵਿੱਚ 21 ਸਾਲ ਤੋਂ ਘੱਟ ਉਮਰ ਦਾ ਕੋਈ ਅਨਾਥ ਬੱਚਾ ਹੈ।
- ਜੇਕਰ ਪਰਿਵਾਰ ਕੋਲ ਖੇਤੀਬਾੜੀ ਵਾਲੀ ਜ਼ਮੀਨ ਹੈ, ਤਾਂ ‘ਫਰਦ’

- Permanent residents of the State of Punjab, except those families having any member who is a regular/regular Government employee in any Department or Board/Corporation/Municipal Committee/Trust/Institution etc. of the Government of India or the State. (Note:- Contract, Work-Charged, Outsourced and Daily Wage Employees in these Institutions can apply)
- Families whose total annual income from all sources of income of all members is less than Rs. 8,00,000
- All families are eligible, except those families that have a regular government employee (contract, work-charged and daily wage employees are excluded)
ਅਪਲਾਈ ਕੌਣ ਕਰ ਸਕਦਾ ਹੈ |
- ਪੰਜਾਬ ਰਾਜ ਦੇ ਸਥਾਈ ਵਸਨੀਕ, ਸਿਵਾਏ ਉਹਨਾਂ ਪਰਿਵਾਰਾਂ ਦੇ ਜਿਨ੍ਹਾਂ ਦਾ ਕੋਈ ਮੈਂਬਰ ਭਾਰਤ ਜਾ ਰਾਜ ਸਰਕਾਰ ਦੇ ਵਿਭਾਗ ਜਾਂ ਬੋਰਡ/ ਕਾਰਪੋਰੇਸ਼ਨ/ਮਿਉਂਸਿਪਲ ਕਮੇਟੀ/ਟਰਸਟ/ਅਦਾਰੇ ਆਦਿ ਵਿੱਚ ਰੈਗੂਲਰ /ਨਿਯਮਿਤ ਸਰਕਾਰੀ ਕਰਮਚਾਰੀ ਹੋਵੇ। (ਨੋਟ:- ਇਨ੍ਹਾਂ ਅਦਾਰਿਆਂ ਵਿੱਚ ਕਾਂਟ੍ਰੈਕਟ, ਵਰਕ-ਚਾਰਜਡ, ਆਉਟਸੋਰਸਡ ਅਤੇ ਦਿਹਾੜੀਦਾਰ ਕਰਮਚਾਰੀ ਅਪਲਾਈ ਕਰ ਸਕਦੇ ਹਨ)
- ਉਹ ਪਰਿਵਾਰ ਜਿਨ੍ਹਾਂ ਦੇ ਸਾਰੇ ਮੈਂਬਰਾਂ ਦੀਆਂ ਸਾਰੀਆਂ ਆਮਦਨੀ ਦੇ ਸਰੋਤਾਂ ਤੋਂ ਕੁੱਲ ਸਾਲਾਨਾ ਆਮਦਨ 8,00,000 ਰੁਪਏ ਤੋਂ ਘੱਟ ਹੈ
- ਸਾਰੇ ਪਰਿਵਾਰ ਯੋਗ ਹਨ, ਸਿਵਾਏ ਉਹਨਾਂ ਪਰਿਵਾਰਾਂ ਦੇ ਜਿਨ੍ਹਾਂ ਵਿੱਚ ਕੋਈ ਨਿਯਮਿਤ ਸਰਕਾਰੀ ਕਰਮਚਾਰੀ ਹੋਵੇ (ਕਾਂਟ੍ਰੈਕਟ, ਵਰਕ-ਚਾਰਜਡ ਅਤੇ ਦਿਹਾੜੀਦਾਰ ਕਰਮਚਾਰੀ ਇਸ ਤੋਂ ਬਾਹਰ ਹਨ)
